    I understand and agree that any information submitted will be forwarded to our office by email and not via a secure messaging system.     Prior To Your Appointment

    1. You will undergo a corona virus screening and will only be allowed to come into the office if you answer "no" to the following questions.  If you answer "yes" to any of these questions, you will be rescheduled 2 weeks after resolution of your situation.
      • Have you had any symptoms of fever, body ache, dry cough, fatigue, chills, headache, sore throat, loss of appetite, loss of smell/taste, diarrhea, nausea, and vomiting for the past 14 days?
      • Have you traveled by plane internationally in the past 21 days?
      • Have you been exposed to someone with these symptoms?
    1. All patients will receive electronic consent forms.  They must be completed prior to your appointment.
    2. Patients entering and leaving the office will be coordinated so that you will not encounter other clients as much as possible.  If you see another patient in the front desk, we ask that you wait outside until that patient is checked-in to their treatment room.
    3. All patients are required to have an appointment.  No walk-ins please.

    As you Enter Our Office

    1. You will be required to wear your own mask at all times except during the procedure.
    2. You will be asked to sanitize your hands by our Patient Coordinator.
    3. We will be taking your temperature with a no-contact thermometer.  If you have a temperature greater than 100 degrees F, we will reschedule your appointment.
    4. Only patients receiving treatment will be allowed inside the office.
    5. There will be no one allowed to wait in our lobby.  You will be escorted to the exam room after your check-in process is completed.
    6. You will be required to wear your own mask at all times except during the treatment.
    7. Prior to any treatment, all patients will read, understand, and sign an informed consent pertaining to receiving an elective treatment during the Covid-19 pandemic.
    8. We will limit exchange of physical items as much as possible and encourage communication via email (info@womenswellness.com) and phone (360-323-2694).
    9. We will be limiting non-essential staff.
    10. Any products purchased from our office will be sanitized.  When possible, we also encourage curb-side pick up or mailing products to your home.
    11. The treatment rooms and any equipment handled by patients will be disinfected thoroughly after every use.

    Employee Guidelines

    1. All staff will be required to stay home if they have any symptoms and stipulations as we are requiring our patients.
    2. All staff will be required to stay home if they have a live-in family member at home infected with Covid-19. 
    3. All staff will have the same screening as our patients at the start of each working day. 
    4. All employees will be required to wear masks while working.
    5. All staff will be within 6 feet of our patients and each other as much as possible. 
    6. All staff will continue, as before, good hand washing practices. 
    7. All staff are required to self-monitor for Covid-19 symptoms and stay at home if they are sick. 
    8. All staff will be encouraged to discontinue non-essential travel.
    9. Regular meetings will be conducted to continue training and education on the most up to date information on on patient and staff safety.  This protocol will likely be updated if necessary based on the latest updates received.
